This was in the latest CS:GO update from Valve:

Fixed a crash on round end/restart or player spawn if a player was parented to another entity that was cleaned up.

Some guy on Reddit made a GTA Vice City map and claims to have a fix for the game crashing on player disconnect. He'll only tell people what it is after his map is released though.
