No way of telling who, at best you could tell when and maybe from which IP, but that's it. No definitive "who".

We use SQL reserved slots (since they're like mini-admins). There's a post in the Sr. Admin forums that has a link to it. Anybody who can access that forum can add reserved slots.

Everything looks pretty good.
I believe the admin/reserved slot/ban lists are all intact.
